·         Child & Teen Services:

·         Child & Family case manager: school consultation/school-based case management, transportation assistance, juvenile court assistance, eligibility benefits and financial supports, behavior management assistance, alcohol and drug treatment as needed

·         Child & Youth Continuous Treatment Team: intensive case management (more face-to-face visits per month) applicable if

·         sudden increase of symptoms

·         multiple incidents of physical aggression

·         child has psychiatric admissions/multiple crises

·         child is at risk for out-of-home placement

·         Child & Family Therapist

·         individual & family counseling

·         Child Psychiatrist/Family Advanced Practice Nurse

·         Assessments and referrals
